Step 1 - Basic & Identity Details: - ID Proof Type and Number - Name, Surname, Father/Spouse Name - Date of Birth (DD/MM/YYYY) with automatic age calculation - Mobile Number, Email, Blood Group, Gender - Fitness checkboxes (Mentally Fit, Physically Fit) Step 2 - Address Details: - Street, Door No, Pincode - Country (dropdown with all options), State (all Indian states), District (auto-populated based on state) - City, Nearest TTD Temple (dropdown) Step 3 - Photo Upload: - Upload a photo (JPEG/PNG, max 1 MB) Step 4 - Volunteer Details: - Qualification, Profession - Employee ID, Designation / Retired as - Specialisation/Skill, Place of Working - Upload Education Document (JPEG/PNG, max 1 MB) Step 5 - Personal Details: - Full Name (as per Aadhaar), Father's/Husband's Name - Marital Status, Alternate Mobile Number - Residential Address Step 6 - Education: - Qualification Category and Specific Qualification (cascading dropdowns) - University/College Name, Year of Graduation, Specialization Step 7 - Profession: - Employment Status, Sector, Profession Category - Role (based on category), Years of Experience Step 8 - Experience & References: - Previously Volunteered (Yes/No) - If Yes: Number of times, Last 2 seva periods, Other Org Name, Experience - Languages Known (multi-select checkboxes) - Two references with Name and Mobile Number Step 9 - Fill the Form: - Click "⚡ Apply to Form" at the top to autofill the entire Srivari Seva application - A "Saved Info" summary card shows your saved details at a glance - Data is auto-saved as you type (debounced) --- Tab 3: 👥 Seva (Group) --- Use this tab to manage and autofill Srivari Seva applications for a group (up to 15 members).
