Not saving prospects information

Good afternoon,
I have had some issues with prospects inputting information into two dots and the information goes in then is removed a few minutes later, we are also seeing on our end one day they are accepted and then the day day the are back into needs follow up, hoping to get some support as we do have a few move ins for this week that are still pending in the system. I was with one of my prospects as she put in her paystubs and saw they uploaded then was removed a few moment slater one of the being Talia Marin

the play screen is normal size. . i cant see the edges because the tppl bars are in the way and too large.same with the home screen.
