Nudge Care is an in-EHR clinical copilot that runs as a side panel inside the practice management system you already use. It reads what is already in the record, including consultation notes, scribed notes, referral letters, lab reports, imaging reports, PDFs and scans, and turns it into diagrams, summaries, searchable clinical context. Then it does the part your EHR does not: it lets you hand that record to the patient, through a portal branded as your clinic. Both halves are included. Neither requires you to change your stack. WHO IT IS FOR Clinicians and clinic teams using Semble or Cliniko as their practice management system. The extension is built around how doctors actually work, and the admin needs of a clinic. WHERE IT RUNS The extension runs only on your EHR, Semble (app.semble.io) and Cliniko (your practice's cliniko.com address). It does not inject into any other website. When you open a patient, the side panel mounts on the right edge of the screen. EHR Functionality 1. Visualise trends Documents, patient profile, scribing notes and consultation notes are analysed to bring structure to your EHR. Biomarkers: pulled from structured blood results or from PDF files, normalised to standard units, flagged against reference ranges and charted over time. Vitals: blood pressure, weight, respiratory rate and others, extracted from documents even when they were never entered as structured fields. Risk indexes: calculated across the whole record and tracked through time: QRISK3, PREVENT, Framingham, TyG, FIB-4, Metabolic Syndrome (5 criteria), eGFR, PhenoAge and HOMA-IR. 2. Search everything Documents with blood results, imaging findings or clinical notes are indexed, labelled, categorised and placed on a timeline, so you can find exactly what you need without opening files one by one. Search inside documents, including PDFs. Ask cross-document questions to surface correlations across the record. Read an AI summary of a document before you open it. Browse the whole record as an organised timeline. 3. Prepare reports Speed up consultation prep and third-party reporting with details drawn straight from the patient's record. Pick a template, set the period you want to cover, and get an editable document in seconds. Download as PDF, copy the text into your EHR, or send it directly by email. Validate where every line of a summary came from. See missing information highlighted rather than silently omitted. Keep a durable record over time of what was discussed. 4. Clinical copilot An assistant you can ask anything about the active patient. It interprets blood work, charts trends inline, summarises the latest consultation in the context of prior ones, suggests next steps with citations, and answers follow-ups. The copilot operates strictly on the active patient and does not aggregate across patients. Portal functionality Nudge Care gives you a portal, included in the plan, not a separate purchase, and set up in clicks rather than a project. Patients leave the consultation with everything: Results, the report, the prescription, the vaccination plan and the next appointment, in one place. Trends explained simply and tracked over time, like blood and urine biomarkers, vitals, so patients can actually understand what changed, not just see a number. Nothing for the patient to download: They get a unique, temporary password by email and open the portal in any mobile or desktop browser. No app store, no account setup, no password reset calls to reception. That is what makes it a portal people open. You decide what a patient ever sees: Every record, summary or update goes through clinician approval before it is published. You validate the data, strip internal notes and noise, and choose what to share. Access is tracked and can be revoked at any time. Getting started in about a minute Install the extension and pin the icon. Open your EHR and click the toolbar icon, the side panel opens on the right. Sign in to Nudge Care with Google, Apple, or your email and password. Create your clinic by adding your EHR API key. Invalid keys are rejected immediately. Invite your team members. Navigate to a patient and click "Import patient". Once the files are parsed, you're ready. Start for free, import a trial account and see the record build itself. DATA AND CONTROL You are the data controller; Nudge Care is the processor. A DPA is presented at clinic creation. Signing out clears your session and per-clinic data immediately. Nudge Care supports clinical decision-making by organising information already held in the patient record. It does not diagnose and does not replace clinical judgement.
