You select a sentence to re-read it, then middle-click a search box or chat input out of habit, and suddenly that sentence is sitting there instead of what you meant to type. Or worse: you highlight a password or API key for some other reason, then middle-click into a public form field and it lands there, visible to everyone. Every other extension that touches middle-click paste solves this by disabling middle-click entirely — which also kills middle-click autoscroll, a feature most people still want.
