Enhanced Markdown viewer with TOC, themes, search, and Mermaid diagram support

View, search, and print Markdown files, change the background theme, and render Mermaid diagrams. This version also adds a table of contents that follows the heading on screen, split-pane edit mode (Apply / Save / Don’t apply), source view, auto-reload, HTML export, font size controls, and an English / Japanese UI (English by default). To open local files, enable “Allow access to file URLs” in the extension options. This is a small tool built for my own use. Please forgive the limited feature set. Markdown形式のファイルの表示、検索、印刷、背景色の変更、および Mermaid 形式の図の表示を行うことができます。 このバージョンでは、見出しから目次を自動生成して本文スクロールに連動させる機能、ソースとプレビューを並べて直す編集モード（反映する／保存する／反映しない）、ソース表示、自動リロード、HTML エクスポート、文字サイズ変更、英語／日本語の切り替え（既定は英語）を追加しています。 ※ローカルファイルを表示する際にはオプションの「ファイルの URL へのアクセスを許可する」を有効にして下さい。 ※自分が利用するための最低限の機能しかありませんのでその点ご容赦下さい。
