Clinical shortcuts for Raqeem: labs, eGFR, screening, chronic, walk-in, family clinic, well baby, pregnancy notes, referral, refill
Clinical shortcuts for Raqeem: labs, eGFR, screening, chronic, walk-in, family clinic, well baby, pregnancy notes, referral, refill Raqeem Bot is an independent browser extension for doctors who use the Raqeem doctor workbench; it is not made by, affiliated with or endorsed by Raqeem's developers. It takes over the repetitive parts of a clinic visit: gathering lab results and the patient's medicines, working out kidney function and cardiovascular risk, entering diagnosis codes, writing routine notes, and placing routine tests and medicines in Raqeem's order grid. You spend less time clicking through screens and more time with your patient. It works inside Raqeem's own screens, through Raqeem's own buttons, forms, services and Save. Nothing is written into Raqeem, ordered or saved until you press the button for it. It supports your clinical judgement; it does not replace it. WHY CLINICIANS INSTALL IT • Fewer clicks per visit: one press does what usually takes several screens. • The right information where you already look: kidney function, cardiovascular risk and overdue screenings appear in the patient file without a search. • Complete, consistent notes: common visits are documented the same careful way every time, with your own saved wording if you prefer. • From note to orders in one press: the walk-in and Family Clinic cases and the well baby and pregnancy tabs write the note, then put tests and medicines in Raqeem's order grid for you to check and save. • Refills within limits: doses, durations and refills follow Raqeem's own limits and your own prescribing rules. • Private by design: patient data stays between you and Raqeem; nothing is sent to the developer or anyone else. WHAT IT DOES Laboratory and risk • Laboratory review: every result since 2021 in one view, grouped by the day the samples were taken, with units, mg/dL conversions, reference ranges, colour-coded flags and trends against earlier results. Export a PDF that names the patient on every page. • Medications: beside the laboratory, the patient's active medicines with how many refills are left (from Raqeem's orders and Wasfaty's dispensing record) and their expired medicines. It only reads; nothing is ordered or changed. • eGFR in the patient banner: CKD-EPI 2021 from the latest creatinine, with its KDIGO stage (G1 to G5) and colour, within seconds of opening the file. • Cardiovascular risk: the AHA PREVENT 10-year ASCVD risk, with each LDL-C result judged against the 2026 ACC/AHA goal for that patient's risk. Screening and diagnosis • Screening: shows the screenings that fit the patient's age and sex and when each was last done, and flashes any that are overdue, including the urine albumin-to-creatinine ratio for patients with diabetes or hypertension. One press enters the ICD-10 code and places the screening tests in Raqeem's order grid for you to complete and save. • Diagnosis shortcuts at ICD 10: obesity coded from the documented BMI, then tabs for screening, chronic illness, referral, the Clinic tab (الزوار), the well baby clinic (under 7), growth (7 to 19 years) and pregnancy (women 12 to 55). Growth and pregnancy appear only for patients they fit. • Mammogram referral: for women aged 40 to 75, refer to Dammam Medical Complex, another hospital, or any primary care centre with a mammogram clinic. The referral form is filled and every value is checked before it is saved. Notes • Chronic illness notes: for diabetes, hypertension and asthma, the latest HbA1c and lipids with a suggested Controlled or Uncontrolled, the Asthma Control Test, and medicines with dose, frequency and timing, written into all four parts of the note in one press. • Family Medicine Appointments (Clinic tab, الزوار): one visit with its type (chronic follow-up, lab results review, lab order, repeat prescription, sick leave or report, pre-employment, premarital, adult vaccination, periodic health check or new complaint), most with its own ICD-10 code, and the conditions you follow up enter theirs. Today's vital signs from the banner and, at a follow-up, the latest HbA1c, lipids and eGFR go into the note. Beside it, 18 long-term cases under five systems, such as type 2 diabetes, hypertension, lipids, thyroid disease, anaemia, vitamin deficiencies, COPD, gout, osteoporosis, depression and anxiety (PHQ-9 and GAD-7) and smoking cessation. A follow-up ticks the tests repeated at every visit; a new diagnosis also ticks a usual first medicine from the region's primary care medicine list. One press writes the note, then puts the tests and medicines in Raqeem's order grid, unsaved. • Walk-in (Clinic tab): 19 common cases under four systems, with a search box: URTI, tonsillitis, otitis media, otitis externa, sinusitis, allergic rhinitis, asthma, conjunctivitis, eczema, scabies, impetigo or cellulitis, gastroenteritis, constipation, reflux, urinary infection, dysmenorrhoea, viral fever, headache and low back pain. Each enters its ICD-10 code and writes the four parts of the note from a few choices, most for adults or children by the banner's age (for example asthma severity from the SpO2). Orders follow the case: most children's liquids dosed by the banner's weight and shown in mL and mg (a few by age, such as lactulose and the cetirizine for allergy or itch), antibiotics ticked by the case and its findings (for tonsillitis by the McIsaac score, for sinusitis only with a bacterial feature), a penicillin allergy respected, and medicines to avoid in pregnancy taken out when you mark the patient pregnant. One press writes the note, then puts the ticked tests and medicines in Raqeem's order grid, unsaved. • Well baby clinic: for children under 7, the visit that fits the child's age from the national immunisation schedule, with the vaccines due, the MOH catch-up rules and the screenings for that age. Growth and development plots weight, length or height and head circumference on the WHO charts with z-scores and centiles, with earlier visits, the corrected age of a preterm baby, the mid-parental height and the CDC milestones for the age. One press enters Z00.1, writes the whole note, then puts what you tick under Orders in Raqeem's orders for you to check: the labs due (CBC, G6PD, Hb electrophoresis), vitamin D drops, paracetamol dosed by weight and saline nasal spray. Children aged 4 to 6 can be referred to an optometry clinic in your health cluster. • Growth (7 to 19 years): the banner's weight and height on the WHO 2007 Growth Reference, each with its z-score, centile and reading, a chart with the earlier visits' measurements, and the mid-parental height with the projected adult height. One press writes the lines into the examination; no code is entered. • Pregnancy (women 12 to 55): dates the pregnancy from the LMP, the ultrasound or both, with the EDD and gestational age; works out gravida, para and abortions; and classes the risk as high, moderate or low, with the VTE risk (enoxaparin dose by weight) and the pre-eclampsia (aspirin) risk. It reads Raqeem's own pregnancy record and can fill Raqeem's pregnancy form for you to save. One press enters the visit's O09 code, writes the four parts of the note, then puts the tests due and the supplements (folic acid 1 mg or 5 mg to 12 weeks, calcium with vitamin D after 12 weeks, iron when anaemic, aspirin when indicated) in Raqeem's order grid, unsaved. You can refer for the dating and anomaly scans, to the moderate or high risk clinic and for antenatal care from 34 weeks, as with the mammogram referral. • My notes: save your own usual wording under any part of a chronic illness, Family Medicine, walk-in, well baby or pregnancy note and add it with a tick. Orders and workflow • Medication refill: repeat the last visit's medicines, or choose your usual ones by group from the region's primary care medicine list (from diabetes and hypertension to well baby and pregnancy), within Raqeem's limits and your rules: metformin follows the eGFR, and an ACE inhibitor and an ARB are never combined. The Clinic, well baby and pregnancy tabs hand it their ticked medicines, and one already saved in the visit is left out and named, so pressing a tab again does not order it a second time. Save all through Raqeem, then see the Wasfaty ID once Raqeem has issued it. • Productivity panel: patients registered, how many came by appointment and how many walked in (each with its share), patients per hour, and discharges against your daily and weekly targets across the centres you work in. • Everyday helpers: earlier vital signs with trends; alert pop-ups closed for you and, if you switch it on, the EMR opened when a patient file opens on Raqeem's Alerts page; M-CHAT answers entered in one press when the parent's answers are all low-risk, and saved only when Raqeem shows a score of 0; "called, not found" (CNF) visits closed in one press; recorded navigation macros; and the browser's Back and Forward buttons stopped inside a patient file, where Raqeem would answer them with a security pop-up (leave the file through Raqeem's own menus). YOU STAY IN CONTROL • Everything happens through Raqeem's own screens, checks and Save buttons, with your existing sign-in. • Text goes into Raqeem's own note boxes, where you can read and edit it, and Raqeem saves it straight away, so nothing is lost when you change page. • Orders, referrals and forms wait for your press, tests and medicines stay unsaved in Raqeem's order grid until you save them, and Raqeem's pregnancy form is left open for your Save. • Raqeem Bot answers Raqeem's own messages only as part of a step you pressed: for example Yes when Raqeem offers to pre-fill the past history, Raqeem's message that a code is already on the visit (so it is not added twice), and the questions on CNF's way to Medical Discharge. It closes alert pop-ups unless you switch this off, and never marks an alert as viewed. PRIVACY Raqeem Bot runs only on the two Raqeem sites, raqeem.anat.sa and moh.raqeem.sa, and its requests go only to the Raqeem site you have open. It has no server, no analytics and no remote code, and it does not sell patient data or send it to the developer or anyone else. Patient information is processed in the open page's memory only, and goes into a file only when you export the laboratory review as a PDF. Only these are kept in your browser, shared by both sites: your macros, your Alerts and productivity settings, your weekly productivity counts (numbers, centre names and your Raqeem user ID), your own note wording (My notes), the clinic you last opened in the Clinic tab, how you add the chronic illness note, and the places you chose for referrals (the optometry centre, and the ultrasound unit, moderate risk clinic and maternity hospital of the pregnancy tab). The Raqeem tab itself holds only a time marker for the laboratory review and, for a CNF run, the visit number (ignored after 10 minutes, gone when the tab closes). WHO IT IS FOR Clinicians with a Raqeem account who work in the Raqeem doctor workbench, on raqeem.anat.sa or moh.raqeem.sa. It does nothing on any other website. NEW SINCE VERSION 2.45 • Works on both Raqeem sites, raqeem.anat.sa and moh.raqeem.sa. • A Pregnancy tab for women 12 to 55: dating, gravida and para, risk, the visit's O09 code, the note, tests, supplements and referrals. • A Growth tab for 7 to 19 years, and Growth and development in the well baby clinic: WHO growth charts, earlier visits, mid-parental height and milestones. • The Clinic tab (الزوار) has 19 walk-in cases and 18 Family Clinic cases, each with its ICD-10 code, note and orders, and the Family Medicine visit has a type, such as a lab order visit. • One press writes the note, then puts the ticked tests and medicines in Raqeem's order grid; pressing again does not re-order a medicine already saved in the visit. • Medication refill groups follow the region's primary care medicine list, with a new Pregnancy group. • The Productivity panel counts appointments and walk-ins, and M-CHAT is filled and saved in one press. Raqeem Bot supports clinical work and saves time; it does not replace clinical judgement. Review every entry before relying on it. Raqeem Bot is an independent tool and is not affiliated with or endorsed by the developers of Raqeem.
