Enhanced FindAGrave search, including Irish counties

This extension enhances the existing search form on the FindAGrave website to to preserve selections when repeating a search with “Refine Last Search” and “Cemetery Lookup” and to allow selection of counties within Ireland. Changes to version 0.0.1.3 from version 0.0.0.2: - Fix cemetery search to re-load last-used selections. - Fix cemetery search to load counties for Ireland and Northern Ireland.
