intake.ninja is a portable, patient-side profile that lives on your device. Enter your information once — name, date of birth, address, insurance, medical history, emergency contacts, practitioners — and the extension fills patient intake forms with a single click. Document images (driver's license, insurance card, signature) are stored encrypted alongside the rest and auto-uploaded into matching file fields when forms ask for them. THE PRIVACY PROMISE • Your data lives only on your device. We have no server. • Your data is encrypted at rest with a key derived from a passphrase only you know. • The extension only acts on the tab you are currently looking at, and only when you click the icon. It has no permission to read your other tabs. Chrome itself enforces these limits. • Nothing transmits automatically. Every action is a deliberate button press. • Before any fill, the extension shows you a preview of every field and value it would write to the page. You confirm before anything is touched. • No telemetry. No usage tracking. No crash reports leave your device. WHO IT'S FOR People who are tired of filling out the same intake form for the hundredth time. People helping a parent or partner navigate medical paperwork. Anyone who has ever decided to seek care and then bounced off the friction of getting registered. HOW IT WORKS 1. Set a passphrase. The extension creates a local, encrypted vault on your device. 2. Enter your profile once across the tabs (Identity, Address, Medical, Insurance, Documents, Care team). 3. When you reach an intake form, click the extension icon and choose "Preview & fill on this page." The extension shows a list of every field it found and the value it would use; you confirm and the form fills. WHAT IT DOES • Encrypted local vault, unlocked by your passphrase. Auto-locks after configurable idle. • Structured patient profile: identity (name, DOB, pronouns, contact), address, multiple emergency contacts, multiple insurance plans, medications, allergies, conditions, pharmacy. • Document storage: driver's license, insurance card front/back, signature. Photos are compressed locally before encryption and auto-uploaded into matching file fields. • Care team: save practitioners with their video-session links (Doxy.me, etc.), client portals, and scheduling URLs. The most-used links appear as one-click buttons at the top of the popup so you never lose a session link mid-day. • Preview-before-fill on every form: see what's about to be written before it lands. • Heuristic field matching that recognizes common field names (first/last name, date of birth, email, phone, address, insurance member ID, etc.). WHAT IT DOES NOT DO • It does not sync, back up, or upload your data. • It does not create accounts or ask for an email. • It does not work without your action — every fill is initiated by you. • It does not have access to any tab unless you click the icon on that tab. WHAT'S COMING • Tailored field mappings for the most-used patient-portal platforms, so harder-to-match fields fill cleanly. • Support for multi-step intake forms (carrying state across pages). • Improvements to camera capture for ID/insurance images.
