Ambient AI scribe for Tebra: drafts the SOAP note, suggests CPT/ICD-10/HEDIS codes, and submits the charge.

Echo Scribe is a clinical documentation assistant for authorized providers at Orion Health and MP Primary Care Services. It works alongside Tebra so providers can spend the visit with the patient instead of the keyboard. What Echo does: Listens during the visit and transcribes the conversation in real time. Drafts a SOAP note (chief complaint, HPI, review of systems, exam, assessment and plan) from the transcript. Suggests CPT, ICD-10 and HEDIS quality codes, each with a short description and the reasoning behind it, for the provider to review. Inserts the provider-reviewed note into the open Tebra chart with one click. Submits the provider-confirmed charge to Tebra as a draft encounter for billing review. Shows today's patients with checkmarks for which notes and charges are already done. Keeps a local backup of recent encounters for 72 hours, so an interrupted visit is never lost. Echo never signs a note or submits a charge on its own. Every note and charge requires the provider's explicit review and confirmation. Code suggestions are drafts to support, not replace, the provider's clinical and billing judgment. Echo can run as a floating panel over Tebra or as a docked side panel. Access requires a practice-issued Echo login. Audio, transcripts and drafts are processed in the practice's HIPAA-covered AWS environment and are shared with Tebra only when the provider chooses to.
