A chart-entry spellchecker that overlays your EMR with coding and documentation suggestions.
Cortex Lens is a chart-entry spellchecker for clinicians. It overlays your existing EMR, reads the chart context, and surfaces coding and documentation issues before sign-off — like a real-time review reading over your shoulder. What it does: • Specificity flags. Highlights unspecified ICD-10 codes that the chart data supports tightening (E11.9 → E11.65 when A1c supports it; I50.9 → I50.31 when systolic HF is documented). • Missing diagnoses. Surfaces conditions implied by labs, meds, or other coded conditions but not on the active problem list. • Quality-gap checks. HEDIS / Star measures (statin in diabetics ≥40, ACE/ARB/ARNI in HFrEF, A1c overdue, etc.). • E/M level evaluation. Reviews the in-progress note against CMS 2021 MDM rules and flags the level the note actually supports plus the single most impactful step to reach the next level. • Audit-risk surfacing. Identifies billed or listed codes the note doesn't adequately support, with a one-sentence justification. Privacy: Cortex Lens reads structured chart context (problem list, medications, labs, demographics) and the in-progress note text from the EMR page you're viewing. It strips patient name and PRN/MRN before sending data to its analysis API. We publish our methodology and benchmarks at cortex-ehr.vercel.app/benchmarks/methodology. Currently supported EMR: Practice Fusion (static.practicefusion.com). Demo site: cortex-ehr.vercel.app/dashboard. Pilot status: This is a pilot release. Formal HIPAA posture (BAA, ZDR with the LLM provider, signed agreement) is in progress. While that lands, please use Cortex Lens with demo or de-identified data only.
