Two diagnosis codes that can't be billed together on one claim under the ICD-10-CM Excludes1 rules, like I12.9 with I15.0. • Side not given. The note says right knee, but the code doesn't: M25.569 → M25.561. • Two codes that should be one. Diabetes and kidney disease coded apart when a combination code covers both: E11.9 and N18.32 → E11.22, with N18.32 kept for the stage. • CKD stage. Hypertensive kidney disease listed without a stage, or two different stages listed at once. • Problem-list cleanup. Duplicate, outdated, or overlapping entries, like E11.9 still listed next to E11.22. • Visit level, when your practice turns it on. Whether the note supports the level you picked, up or down. Flags go both ways. If the note doesn't support a code or a level, Lens says so. How it works 1. Open a patient's encounter in your EHR. A Cortex box appears at the top right. 2. Click Analyze, or turn on Auto-analyze to check every visit automatically. 3. Check each flag against the note. If a change is needed, you make it in your EHR, and Lens checks again. Your team stays in control • Read-only. Lens never edits your chart or submits a claim. • People decide. Your biller or provider makes every change. Diagnosis questions go to the provider. • Names stay behind.
