Circle Health

Circle Health is an internal browser extension for Circle Health's chronic care management (CCM) team. It runs inside the EHR systems our care coordinators already use (MDland, eClinicalWorks, Elation, and Practice Fusion) and streamlines identifying and enrolling patients who qualify for CCM. What it does: Reads the ICD-10 diagnosis codes on the patient chart currently open in the EHR Automatically checks CCM eligibility using Circle Health's qualification rule (2 or more distinct, valid chronic diagnosis codes) Surfaces an enrollment panel directly on the patient's chart when they qualify, so staff can review and select the relevant diagnosis codes Submits enrollment to Circle Health's internal CCM platform, authenticated through the staff member's existing Circle Health session (a legacy API token is supported as a fallback) The extension only activates on the specific EHR domains listed in its permissions. It does not collect, sell, or share patient data with any third party, all data stays within Circle Health's own systems and is used solely to support CCM enrollment workflows. This extension is intended for use by authorized Circle Health staff only.
