Open the ACGME case entry website using the link on the extension, select the relevant options using the streamlined form, and click Fill and Submit. The author has NO affiliation with the Accreditation Council for Graduate Medical Education (ACGME). Privacy Notice: No user data is ever collected or transmitted. Please note: - While every effort has been made to ensure accuracy, it is your responsibility to verify your data is entered correctly. - The extension may stop working if the ACGME updates their website. - The extension will not fill your Site and Supervisor. Consider creating a template with your hospital and program director's names to automate this process.
